How do you earn rewards?
You can earn rewards for completing preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
There is no extra cost for preventive care. Better yet, we reward you for taking steps toward health.
Earn gift cards
You can earn gift cards for each healthy step you complete. So it really does pay to check on your health.
Screenings
- [$15: Get your mammogram every year from ages 54 to 74 to screen for breast cancer
- $50: Complete all diabetes tests (A1C, LDL and retinal dilated eye exam) from age 21 and up for members with diabetes]
Wellness visits
- [$30: Complete all wellness visits (birth to age 15 months)
- $15: Complete your wellness visit once a year (ages 2 years +)]
Vaccines
[$5: Get your flu shot once a year]
Pregnancy and birth care
- [$50: Complete the notice of pregnancy (PDF) in your first trimester
- $25: Go to your first prenatal visit with your provider
- $10/visit: Complete up to 10 follow-up prenatal visits (after your first one)
- $50: Complete all progesterone shots (only for members at high risk of premature birth)
- $50: Go to your postpartum visit with your provider up to eight weeks after birth]
Medications

How to get your gift card
[Example 1: Once you’ve completed each healthy step, you’ll want to call us to claim your gift card.]
[Example 2: Your provider will submit a claim for your completed screening, annual visit or care. A claim is a form your provider sends us. That’s how we know you went for a visit. Once we receive the claim, we’ll send your gift card letter in the mail. Look for your gift claim code and instructions in that letter. We’ll send it within 90 days of receiving your provider’s claim.

Just remember, you won’t be able to use gift cards for products that can cause health risks, like alcohol, nicotine products or firearms.
