Once your [Medicaid/name of state’s Medicaid plan] application receives approval, you can choose us as your health plan. If you don’t choose a plan, the state will choose one for you. Already enrolled in a health plan? You can change plans at certain times during the year.

Why choose us?

Choose Aetna Better Health as your [Medicaid/name of state’s Medicaid plan] plan. As a member, you’ll get extra benefits that go beyond your standard health coverage. Here are a few of the extras you’ll receive:  

  • [Rewards you can earn

    Get rewards for focusing on your health. You can start earning gift cards for completing healthy activities, like wellness checkups, yearly screenings, pregnancy care and more.]

  • [$25 for OTC products

    Spend $25 on over-the-counter (OTC) health supplies from CVS Health®. We'll send them to you every month at no extra cost.

  • [Adult dental and vision benefits

    Members (age 21 and older) get up to $1,000 per year toward a dental exam and cleaning twice a year. Fillings, extractions and X-rays may also be included. Adult dental vision benefits include up to $150 toward glasses or contacts, as well as one eye exam per year.]

  • [Help with rides

    Get help with rides to health care visits with stops at the pharmacy for medicine. Are you 18 or older? If yes, you can also get up to 20 one-way rides each year at no cost. These rides are for job interviews, grocery shopping and other reasons.]

Choose or change health plans

Remember to choose Aetna Better Health of [state] when it’s time for enrollment. You have a few options:
 

  • [Online: Visit [Medicaid plan name] enrollment.

  • By phone: Call [Medicaid plan name] at [X-XXX-XXX-XXXX]. You can call [plan name’s phone center hours]. Tell them you choose Aetna Better Health of [State] as your plan. 

  • By mail: Complete the enrollment form you got in the mail. Send it to the address on the form.] 

[Switching to Aetna Better Health]

Already enrolled in a health plan? [You can switch to Aetna Better Health during open enrollment. This period is 60 days long and happens once a year. Before then, you’ll get a letter with all the info you need to change plans.]
