Do you need help getting care?

Are you or your child a STAR Kids member? If so, a service coordinator can help you get care. We may have assigned you a service coordinator when you enrolled with Aetna Better Health® of Texas. If not, you can ask for one.

Just call Member Services at 1-844-STRKIDS (1-844-787-5437) (TTY: 711). We’re here for you 24 hours a day, 7 days a week. Already have a service coordinator? Reach out to them for help.

What does a service coordinator do?

Service coordinators work with you or your child to get the right care from birth to age 20. They’re your go-to person for health and community-related needs through the life of your plan. Your service coordinator can help with:

 

Personal support

 

  • Find out your needs and work with you on a plan of care, with actions and goals
  • Screen to decide if you need more help right away 
  • Stay in touch to check up on your health and track your service plan

Resources

 

  • Help you connect with services (covered and noncovered) 
  • Find qualified providers and therapists and talk with them about your care

Education and planning

 

  • Share education and resources for waivers 
  • Help with discharge planning during and after a stay in the hospital or other facility 

Prepare for the future

STAR Kids benefits apply through age 20. Before that, a transition specialist can help prepare adolescent and young adult members for adulthood. They may also work closely with your providers and support network.

 

Transition specialists can help you:

 

  • Get ready for transitioning to adulthood
  • Provide education to help with self-management
  • Apply for community services and resources
  • Find future jobs and job training
  • Find providers for adult care

    • Learn more about your diagnosis and the disease process and progression.
    • Identify alternative caregiver, if needed.
    • Coordinate with school and IEP to ensure consistency with goals.
    • Provide options for career planning and higher education opportunities.

    • Discuss ways to begin managing your own medical care.
    • Provide guardianship information, and alternatives to guardianship.
    • Start looking for adult providers. (PCP, specialists, dentist)
    • Discuss current and possible future living options.

    • Verify eligibility for Supplemental Security Income (SSI) if not already in place.
    • Provide guardianship information, and alternatives to guardianship.
    • Start looking for adult providers. (PCP, specialists, dentist)
    • Discuss current and possible future living options.
    • Assess the need for new adaptive aids/equipment due to growth and development.

    • Assess the need for new adaptive aids/equipment due to growth and development.
