Get rewards for focusing on your health. You can earn rewards by completing healthy steps, like annual screenings, wellness exams and more. It’s one reward per year for each topic unless the description says otherwise.

You can earn rewards for living a healthy life. You’ll earn rewards on a gift card after you complete one of the healthy steps on this page. You can use your gift card to shop in store or online at retailers who take part in the program.

How to register for rewards

Register for the rewards program through the Aetna Medicaid portal Choose the option to redeem the rewards you earn. When you do, you’ll receive a gift card at the address you provided.

 

Rewards load onto the same card each time you complete a qualifying healthy step. You can activate your gift card rewards* and check for rewards you already earned in the Aetna Medicaid portal.

Rewards for getting care

Here are some rewards you can earn for healthy steps. Learn more about these rewards later on this page.

  • Health Allowance

    $25 for annual visits

    Earn $25 when you complete your annual well-care visit for adults and children over age 3. 

  • Up to $100 for pregnant moms

    Earn $25 for a prenatal visit, $25 for notification of pregnancy and $50 for a postpartum visit. 

  • Up to $80 for babies

    Earn $10 per well-child visit (up to 8 visits) during the first 30 months of life.

  • Up to $115 for immunizations

    Earn $55 for completing childhood immunizations. Earn up to $60 for adolescent immunizations.

  • Up to $50 for cervical cancer screening

    Earn $25 when you complete a cervical cancer screening. Earn another $25 when you do so by December 31, 2025.

  • Up to $50 for diabetes care

    Earn $25 each for Hemoglobin A1C (HbA1c) control and  blood pressure control.

  • Up to $50 for behavioral health follow-up

    Earn $50 when you complete a 7-day follow-up visit ($25) and a 30-day follow-up visit ($25).

  • $20 for health risk screening

    Earn $20 when you complete your health risk screening within 60 days of enrollment.

Earn rewards in 2025



Here are all the ways you can earn rewards on your gift card in 2025.

 

Effective dates: January 1, 2025 to December 31, 2025

  • Annual well-care visit (PCP): Earn $25 when you complete the visit. This reward is for people ages 20 and older.

     

    Annual well-child visit: Earn $25 when you complete this visit. This reward is for people ages 3 to 21.

     

    Well-child visits: Earn $10 with eight well-child visits in the first 30 months of life:
     

    • 6 or more visits in the first 15 months of life

    • 2 more visits by 30 months

  • Cervical cancer screening: Earn $25 when you complete the screening. This reward is for:
     

    • Women ages 21 to 64 who have a Pap test once every 3 years
    • Women ages 30 to 64 who have a (human papillomavirus) HPV test or HPV/Pap test co-testing once every 5 years

     

    Breast cancer screening: Earn $25 every 2 years when you complete the screening. This reward is for women ages 50 to 74.

  • Earn $25 for HbA1c control (less than 8%). This reward is for people ages 18 to 75 with a diagnosis of type 1 or type 2 diabetes.

     

    Earn $25 for blood pressure control: This reward is for people ages 18 to 75 with both:
     

    • Systolic blood pressure below 140 mm Hg
    • Diastolic blood pressure below 90 mm Hg

  • Earn $55 total for certain childhood immunizations. 

     

    Earn $30 when children get these vaccines by their 2nd birthday:
     

    • Four diphtheria, tetanus and acellular pertussis (DTaP)
    • Three inactivated polio vaccine (IPV)
    • Three hepatitis B (HepB)
    • One measles, mumps and rubella (MMR)
    • Three Haemophilus influenza type B (Hib)
    • One chicken pox varicella-zoster virus (VZV)
    • Four pneumococcal conjugates vaccine (PCV)

    Earn another $25 when children get all the above vaccines, plus these vaccines, by their 2nd birthday:
     

    • One hepatitis A (HepA)
    • Two or three rotaviruses (ROT)
    • Two inactivated influenza vaccine (IVV

  • Earn $20 per vaccine (max $60) for members who get these vaccines between ages 11 to 13:
     

    • One dose of meningococcal vaccine
    • Tetanus, diphtheria & acellular pertussis vaccine (Tdap)
    • Two- or three-dose human papillomavirus vaccine (HPV) series by age 13 (counts as one reward event)

  • Health risk screening: Earn $20 when you complete screening within 60 days of enrollment by paper, online or by phone.

  • Blood pressure less than 140/90: Earn $25 for blood pressure control with a diagnosis of high blood pressure. This reward is for people ages 18 to 85.

  • Annual child dental exam: Earn $20 when you complete a dental checkup. This reward is for people under age 21.

  • Follow up after a hospital stay for mental health:
     

    • Earn $30 for a 7-day follow-up: ages 18 and older
    • Earn $20 for a 30-day follow-up (8 to 30 days): ages 18 and older

     

    Follow up after emergency department visit for alcohol or drug use:
     

    • Earn $30 for a 7-day follow-up: ages 18 and older
    • Earn $20 for a 30-day follow-up (8 to 30 days): pages 18 and older

     

    Follow up after treatment for substance use:
     

    • Earn $30 for a 7-day follow-up: ages 18 and older
    • Earn $20 for a 30-day follow-up (8 to 30 days): ages 18 and older

  • Postpartum visit: Earn $50 when you complete the visit 1 to 12 weeks after delivery.

     

    Prenatal visit: Earn $25 when you complete the visit within one of these:
     

    • The first trimester 
    • 42 days of enrollment

    Notification of pregnancy: Earn $25 when you tell us about your pregnancy by the end of the second trimester.
