This is an option for you to receive certain notices or communications electronically. You or a designee have the option to receive some of these notices or communication by email. The notices you can receive by email may be
Notices about:
- Services you asked for
- Services you are getting
- Plan appeals
- Complaints; and
- Complaint appeals
Other communications about:
- Your member handbook
- Our provider directory
- Changes to your Medicaid managed long term care benefits;
Your rights and preferences
You have the right to choose a designee. This is someone you choose to get your determination notices. If you choose a designee, we’ll provide all your notices to that person. And we’ll follow all federal and state laws, rules and info on this page. Then, all requirements for electronic notices will apply to them, the same way they apply to you.
Some laws and rules say we must provide determination notices in writing. You can choose the way you want to get these notices.

Facts to know about determination notices
- You can change your preferred way to get notices anytime.
- If you don’t choose a preferred way, we’ll use the regular nonelectronic way that laws and rules require:
- A phone call
- Written notice by mail
- You can ask us to provide another way to get notices if you have a disability or language need.
- We may not require you to choose an electronic way to get notices.
- We may not retaliate, discriminate or treat you differently based on your preferred way to get notices.
When you choose an electronic way to get notices, we’ll provide the info:
- In a readily accessible way
- In a format that you can save and store electronically, as well as print
You can ask us at any time to send any electronic notice in paper form through regular mail. Or get another format if you have a disability or language need.

Starting January 1, 2025, you can use the SCN to connect to community programs that offer support for things, like housing, rides, care and more.
Through the SCN, you’ll meet with a Social Care Navigator who will:
- Check your eligibility for services that can help with your health and well-being
- Ask questions to understand where you might need extra support
Services and offerings
If you qualify for services, the Social Care Navigator can work with you to get the support you need. You may qualify for more than one service, depending on your situation. These services may include:
- Housing and utilities support:
- Installing in-home equipment like ramps, handrails and grab bars to make your home accessible and safe. Repairing and fixing water leaks to prevent mold from growing in your home.
- Sealing holes and cracks to prevent pests from entering your home.
- Providing an air conditioner, heater, humidifier or dehumidifier to help improve air flow in your home.
- Helping you find and apply for safe and stable housing in the area.
- Transportation services:
- Helping you with rides, including public or private transportation, to places approved by the SCN (examples: job interviews, parenting classes, housing court to prevent eviction, and city or state department offices to get important paperwork).
- Care management services:
- Getting help with finding a job or job training programs, applying for benefits, managing your money and more.
- Getting connected to services like childcare, counseling, crisis support, health homes programs and more.
